JobKeeper Nomination Form - Employee
You can forward the Employee Nomination Form to all of your eligible employees to complete. Please ensure they read the nomination form in full and sign where indicated.
You must receive the completed Nomination Form back from each employee before 30 April 2020 and before proceeding any further if you wish to make a claim for April 2020. If you are claiming for a period commencing after 1 April 2020, the completed nomination forms must be received prior to the last day of the relevant period.
Please note this Form must be kept for 5 years.
